Laser treatment options

In addition to eye laser treatments, LASIK or LASEK, there are other surgical options for correcting refractive error. Depending on the severity and type of refractive error, or due to certain other contraindications, a laser treatment might not be possible or advisable, e. g. if the cornea is too thin or if the refractive error is too large. Rather than changing the corneal surface as in the laser treatment, during lens-surgery a lens is implanted in the eye. Artificial lenses are used either instead of the eye’s own lens (lens replacement) or in addition to it (lens implant).

Lens surgeries thus allow people with a very high refractive error or presbyopia to enjoy a life without glasses.

General information about the laser treatment of the cornea

Depending on the individual condition of your eye and the process employed, a myopia up to -10 diopters, astigmatism up to +/-5 diopters and hyperopia up to +5 diopters may be treated.

Moreover, we offer a special laser correction for improving presbyopia (“Presbylasik”) to patients from 45 upwards.

We conduct a thorough preliminary examination to determine whether your eyes are fit for a laser eye treatment. Only then shall the doctor personally recommend to you a suitable eye treatment method.

Femtolaser (Z-Lasik)—accuracy, security and freedom from pain

Until recently, during LASIK treatments, a thin layer (“flap”) of the cornea was cut and opened with a special “mini-plane”, called a microkeratome. In modern, up-to-date Femtolasik technology applied at our Center, the femtosecond laser replaces the microkeratome, and therefore allows for the first time the LASIK treatment to be performed completely without the use of mechanical devices—hence the term “all-laser LASIK”.

After the laser-guided preparation of the flap (no cut), a piece of the superficial corneal layer is flapped open and the cornea is re-sanded just as with LASIK treatment, using an excimer laser to correct the refractive error.

The Femto-LASIK treatment is especially suitable for patients who previously have had reservations about the mechanical cutting of the cornea with a microkeratome and who rather prefer the high precision, safety and freedom from pain of the laser cut.

The blade-free Femtolaser method used by us, otherwise knows as Z-LASIK, is the most advanced development of the blade-free method. In the Eye Laser Center Graz, we employ the Femtosecond laser from Ziemer, currently the fastest and most accurate femtosecond laser on the market.

LASIK (laser in situ keratomileusis) has been performed since 1993 as the currently most widely used method for correcting vision defects. Accurate and stable results are achieved by the combination of a microsurgical cutting technique and particularly, the tissue evaporation by the excimer laser.

In the LASIK method with an excimer laser (Cold Laser) corneal tissue is removed, so as to correct the refractive power. The procedure is virtually painless, as the doctor prepares before the start of laser treatment the cover of the corneal surface, which ends up as a “natural” band-aid and later covers and thus protects the eye. The surgery is a thorough outpatient procedure as the eye is anesthetized with drops only! The entire procedure takes about ten minutes per eye, whereas, thanks to our advanced laser system, the pure laser treatment lasts an average of 5-10 seconds.

“Surface-Ablation” (“C-Ten” / trans-PRK / PRK / LASEK / EPI-LASIK)

During the so-called “surface ablation” (treatment of the surface of the cornea), after removal of the re-growing, superficial corneal epithelium, like in LASIK, also stromal corneal tissue is removed with an excimer laser to correct the refractive power.

Advantages

  • Applicability: A safe laser correction becomes possible for thinner corneas (500μm)
  • Safety: Since there are no flaps to be cut, the Femtolaser is by far the safest eye laser method.

Disadvantages:

  • Slightly more painful after surgery than after LASIK

The removal of tissue occurs with LASEK on the surface of the cornea after the top layer of the cornea (epithelium) has been removed either with a small rotating brush or by the laser (trans-Epithelial surface treatment or trans-PRK). There is no evidence that a putting back of the epithelium is beneficial. The scraped epithelial cells are discarded. For protection, the doctor uses a bandage contact lens, which after 1–2 days, when the epithelium is healed, is removed. After surface treatment, the eye feels a little rough for 1–2 days but due to our very effective pre-and post-operative pain medication, severe pain is extremely rare.
